Medical Forms:
- Class I/II Medical From
- Class I: Outings less than 72 hours and activities similar to home and school.
- Class II: Outings greater than 72 hours and activities similar to home and school.
- Class III Medical From
- Adults 40 and over
- Strenuous activities such as athletic competition, adventure challenge or wilderness expedition.
