Every Scout and adult who participates in any scouting event of up to 72 hours duration must have a current Class I Medical form on file with the unit. The requirement for this information is a key part of keeping scouting safe.
This form is to be immediately available at all events. It does no good to have the form in someone drawer and you are out camping.
A Class I form has very basic, but useful medical history, contact and insurance information.
Prior to an event you, as the leader, will know of any condition which may require special consideration and give you the opportunity to review this information with the parent or guardian before the event.
If a problem should arise at the event, you will have the relevant information needed by you and emergency personnel. In addition, you will have needed contact information to reach the parents or guardians.
Yes, adults need to fill out the same Class I form. If you are over 40 you will need to get a Class II form which requires a visit to your doctor.
Scouts and adults participating in events which are of longer duration than 72 hours, require a Class II medical form.
The Class II medical form has more extensive information. Going to summer camp requires a Class II medical form.
Forms are available here or you can pick them up at either of the Council offices.
These forms are kept as part of the unit records and must be available on the outing. Many units make a copy to carry in their scout trailer.
It's important to keep the forms up to date. Class I forms must be filled out annually and the Class II form is good for 3 years. Adults who are over 40 must update their Class II forms annually.
Be sure to include the current Class I form for all participants in your file.
If a medical form is needed for a district or council event, don't submit the original. Make copy and retain the original in your unit records.
